I have been trying to help him out with his diet for a few months now, and nothing seems to work with him. Assuming that he is counting his macros correctly, I don't see any reason for him not to be losing weight on his current diet plan.

My question is, could there be an underlying medical condition that is preventing him from losing weight?

If he is 180 I would have him drop calories to at least 1800 (with 1 or 2 high calories/refeed/skipload type days each week) to see if that doesn't do something. for someone weighting 180 at 18% BF (roughly 150lb lean mass) 2100 calories might be a bit high IMO.

If you think it will work or at least better gauge if something is wrong im more then willing to give it a go. Could you detail what exactly i should do. The refeeds and etc im unclear on. Or at least provide a guide. Please and thank you. If something were to be wrong. What would you all suggest i do. I can tell you my pcp has not been much help. She did the thyroid and the sleep study. After that she told me quite literally to just deal. Like who would i contact and what questions could/should i ask

I would adjust the calories a little bit first. Then if you still aren't making progress in 4-6 weeks being consistent with everything I would try to find a different PCP and have them run a complete blood panel to get a more complete picture of all your hormones.

I don't know anything about your diet other than the calories, so a more detailed explanation of macro's, what your meals consist of etc. would help. Are you logging everything, and measuring everything?

Not knowing anything else I would tell you to drop your calories to 1800 6 days/week, then on one day each week bump them up to 3000-4000 with lots of carbs that day.

The keto diet im on calls for under 30g carbs and about 1 to 1.5 g protein daily the rest of cals from fat. I already have a 1 day a week carb cycle general i just eat large portions of brown rice with soy sauce. And if i intend to go out ill have beer on those days. Ill lower the cals back for a while and see where that takes me. Is there a specialist for this type of thing that be more sympathetic for this situation? I really would like to steer clear of general practitioner's.

You typically need a referral to see a specialist, but I would guess you want to see an endocrinologist... it very well could be that nothing is wrong and you need your calories lower.

Also am I understanding this correctly that you are only eating 1-1.5g of fat per day/per lb bodyweight or what?

Can you give an exact breakdown of what you eat each day, macro's, etc?

How often are you drinking beer, how much brown rice is eaten on high carb days?

If you don't have exact answers to this it's hard to say where exactly you are going wrong.

In my experience metabolic disorders that actually prevent you from losing weight are pretty freaking rare.

ONo i meant 1 to 1.5 grams of protein. But i have my list at home so ill copy it over when i get off work

If anything im probably way under my macros now. I was doing everything as keto friendly shakes but that gets miserable quickly. So i started doing whole foods. Like just today i had a rotisserie chicken breast. With hot sauce, for snacks i have a serving and a half of natty peanut butter, lunch a half a cup of tuna salad with sugar free relish over a lettuce leaf, another snack of pb, dinner will probably be some more chicken. After workout will be two scoops of gold standard whey in water and a server of creatine, later snack pb again.

Also taking 3 doses of eca daily

For a Keto diet that doesn't look half bad. However from the brief description of some of your daily food intake would indicate that your carbs are much higher than 16g, and probably even higher than 30g which would knock you out of ketosis more than likely. Also you said you drink beer on occasion, so your macros can't be "daily" for 3-4 months.

Give a detailed breakdown of measured daily meals, as accurately as possible.

I don't think anything is medically wrong... you just need to be more diligent with measuring your food intake, sticking to your macro's, and likely decrease your calories a little bit. Also on your high carb days I might suggest something a bit "dirtier" with a higher sugar/simple carb content.

The beer is only on carb up days. Would maybe something like cereal or oats work better for carb up, the brief description of recent eating(the one where i described tuna, and natty peanut butter) thats just been the last 3 weeks. The carbs arnt above 30g, for instance the 3 snacks of pb are 18g total of carbs, the relish i use in the tuna salad is carb free, but even with say the lettuce and tuna and protein shake i doubr im above an additional 12g carbs. And i know the calories are lower. Then what i was doing with the shakes. I didnt record the cals and fat as perfectly as i did before since i was more or less doing it as a break from shakes. I figured as long as the carbs and protein are on point then the rest being lower then my macros wont hurt too much. With the shakes i could lower my cal count easily. Keep in mind im literally only eatinf shakes when doing it that way. You tend to start having bowel issues. All i have to so is cut back on the fat i add to the shakes. I use vegetable oil to add fat to the shakes. And for the whey i use dynatec.
